We are a Medicare Participant Provider and accept assignment. This does not mean the entire visit will automatically be covered by Medicare. When a diagnosis of screening is used for a visit or procedure Medicare will not cover. Therefore, we will ask the patient to sign a waiver stating they understand they will be responsible for any amount not covered by Medicare or secondary insurance.
Our Doctors participate in most private HMO plans as Gastroenterology Specialists.
Most insurance plans are accepted-please inquire about your plan.* We file insurance claims and as a courtesy, we will file your secondary insurance if applicable.
If you participate in a HMO/Managed Care Plan, please read the following:
HMO/Managed Care plans require an authorization to see a specialist. It is your responsibility to acquire the initial authorization for your visit and subsequent follow up visits. We request that you bring that with you at the time of your visit or make arrangements with your Primary Care Physician to have it faxed to our office. Our authorization person will handle all authorizations for procedures.
Our Doctors are NOT Medicaid Providers, if you have Medicaid as a primary or secondary, you will be responsible for payment of all visits and procedures. |
